Referential of clinical training for health professionals

This booklet contains the documentation on the supervision of learners in clinical placements. In addition to the introduction, it contains 5 chapters: the structure of the training center and the learner, the responsibilities of the actors, the evaluation of the internship, the post-internship follow-up and the discussion. The structure to be selected as a training center must obtain at least 70% of the accreditation evaluation, 60% of the accreditation and have competent personnel including clinical referents who will work in close collaboration with the pedagogical referents.Continuing education aimed at reinforcing skills; the administration of pre- and post-tests (memorization) to evaluate the achievement of objectives is not recommended. On the other hand, co-assessment in situ to establish a final skills assessment and evaluation of the impact of the course is recommended. This approach has advantages and some limitations that should be taken into account when testing it. These are the high cost, the lack of resource materials and the lack of clinical competency frameworks. These limiting factors should not be an expense.

Este folheto contém a documentação sobre a supervisão dos alunos em colocações clínicas. Para além da introdução, tem 5 capítulos: a estrutura do centro de formação e do aluno, as responsabilidades dos intervenientes, a avaliação do estágio, o acompanhamento pós-colocação e a discussão. A estrutura a ser seleccionada como centro de formação deve obter pelo menos 70% na avaliação da autorização, 60% na acreditação e ter pessoal competente, incluindo referências clínicas, que trabalhará em estreita colaboração com as referências pedagógicas.A formação contínua destinada a reforçar as competências; a administração de pré e pós-testes (memorização) para avaliar a realização dos objectivos não é recomendada. Por outro lado, recomenda-se a co-avaliação realizada in situ para o estabelecimento de uma avaliação final de competências e avaliação do impacto do curso. Esta abordagem tem vantagens e algumas limitações a ter em conta ao testá-lo. Estes são o elevado custo, a falta de recursos documentais e a falta de repositórios de competências clínicas. Estes factores limitantes não devem ser um custo.

Cette brochure reprend la documentation de l'encadrement des apprenants reçus en stage clinique. En plus de l'introduction, elle compte 5 chapitres: la structuration du centre de formation et l'apprenant, les responsabilités des acteurs, l'évaluation du stage, le suivi post stage et la discussion. La structure à retenir comme centre de formation devra obtenir au moins 70% à l'évaluation de l'habilitation, 70% à l'accréditation et disposant d'un personnel compétent dont les référents cliniques qui travailleront en étroite collaboration avec les référents pédagogiques.La formation continue visant le renforcement des compétences; l'administration du pré et post test (mémorisation) pour évaluer l'atteinte des objectifs est déconseillée. Par contre, la co-évaluation réalisée en situation pour l'établissement d'un bilan des compétences final et l'évaluation de l'impact du stage sont recommandées. Cette approche présente les avantages et quelques limites à prendre en compte lors de son expérimentation. Il s'agit du coût élevé, de l'insuffisance des ressources documentaires et des référentiels des compétences cliniques. Ces facteurs limitants ne devraient pas constituer un frais.

Management of pulmonary bullous dystrophies

Bullous dystrophy or pulmonary emphysema bulla is a distension greater than 1cm of an airspace located beyond the terminal bronchioles. The aim of our study was to describe the indications and evaluate the results of surgery for bullous dystrophy. It was a retrospective, cross-sectional, longitudinal study from January 01, 2012 to August 30, 2020. The study included 84 patients operated for bullous dystrophy. Bullous dystrophy accounted for 1.6% of hospitalizations during the period. The mean age of the patients was 39.6 ± 23.4 years, with extremes of 0.16 years and 85 years. Male gender was predominant in 79.8% with a sex ratio of 3.94. Primary or idiopathic bullous emphysema accounted for 21.4%. Chest X-ray and CT scan were performed in almost all cases; and allowed to make the diagnosis. In our study, TPL was the most performed procedure (65.5%), axillary thoracotomy (3.6%), and video thoracoscopy (1.2%) was performed during a bilateral one-step thoracic approach.
