Investigación de intervención para la gestión del cambio
En el ámbito de la salud, las nuevas recomendaciones y métodos de atención evolucionan constantemente y a veces son difíciles de aplicar. Para tratar de implementarlas en forma de protocolos de atención, en el contexto del destete precoz de los pacientes intubados ventilados en las unidades de cuidados intensivos, recurriremos a las Ciencias de la Educación para que nos ayuden en este proceso de gestión del cambio y de apoyo, gracias a los actores de referencia del sector. Recurriremos a varios autores para aportar una perspectiva epistemológica, que pondremos en tensión con la doble postura del profesional-investigador para llevar a cabo una investigación-intervención. Los resultados de la encuesta exploratoria monocéntrica revelan que la agitación de los pacientes intubados ventilados induce ansiedad en los cuidadores. El protocolo y las prácticas de destete no están armonizados entre las dos unidades de rehabilitación. Una de las principales vías de acción que el grupo de actores de referencia está considerando sería la implementación de una herramienta de destete protocolizada para armonizar las prácticas de atención en las dos unidades en beneficio de los pacientes y los cuidadores.

Investigação de intervenção para a gestão da mudança
No campo da saúde, novas recomendações e métodos de cuidados estão em constante evolução e por vezes difíceis de implementar. Para tentar implementá-los sob a forma de protocolos de cuidados, no contexto do desmame precoce de pacientes entubados ventilados em unidades de cuidados intensivos, recorreremos às Ciências da Educação para nos ajudar neste processo de gestão e apoio à mudança, graças aos actores de referência do campo. Apelaremos a vários autores para fornecer uma perspectiva epistemológica, que colocaremos em tensão com a dupla postura do praticante-pesquisador, a fim de realizar uma intervenção de investigação. Os resultados do inquérito exploratório monocêntrico revelam que a agitação de pacientes entubados e ventilados induz ansiedade nos prestadores de cuidados. O protocolo e as práticas de desmame não estão harmonizados entre as duas unidades de reabilitação. Uma das principais vias de acção que o grupo de actores de referência está a considerar seria a implementação de um instrumento de desmame protocolizado para harmonizar as práticas de cuidados nas duas unidades, em benefício dos doentes e dos prestadores de cuidados.

Research-intervention in support of change
In the health field, new recommendations and methods of care are constantly evolving and sometimes difficult to implement. In order to try to implement them in the form of care protocols, in the context of early weaning of intubated patients ventilated in intensive care units, we will call upon the Sciences of Education to help us in this process and to accompany the change thanks to reference-actors from the field. We will call upon several authors to have an epistemological viewpoint, which we will put in tension with the double posture of the practitioner-researcher to carry out a research-intervention. The results of the exploratory monocentric survey reveal that the agitation of ventilated intubated patients induces anxiety in the caregivers. The weaning protocol and practices are not harmonized between the two rehabilitation units. One of the main avenues for action that the group of referring actors is considering is the implementation of a protocolized weaning tool to harmonize care practices in the two units for the benefit of patients and caregivers.

Per cercare di implementarli sotto forma di protocolli di cura, nel contesto dello svezzamento precoce dei pazienti intubati ventilati nelle unità di terapia intensiva, faremo appello alle Scienze dell'Educazione per aiutarci in questo processo di gestione e supporto del cambiamento, grazie agli attori di riferimento del settore. Faremo appello a diversi autori per fornire una prospettiva epistemologica, che metteremo in tensione con la doppia postura dell'operatore-ricercatore per realizzare una ricerca-intervento. I risultati dell'indagine esplorativa monocentrica rivelano che l'agitazione dei pazienti ventilati intubati induce ansia nei badanti. Il protocollo e le pratiche di svezzamento non sono armonizzati tra le due unità di riabilitazione. Una delle principali vie d'azione che il gruppo di referenti sta considerando sarebbe l'implementazione di uno strumento di svezzamento protocollato per armonizzare le pratiche di cura nelle due unità a beneficio dei pazienti e di chi li assiste.

Interventionsforschung im Dienste der Begleitung von Veränderungen
Im Gesundheitsbereich werden ständig neue Empfehlungen und Pflegemethoden entwickelt, die manchmal schwer umzusetzen sind. Um zu versuchen, sie in Form von Pflegeprotokollen im Rahmen der frühzeitigen Entwöhnung intubierter und beatmeter Patienten auf der Intensivstation umzusetzen, werden wir uns auf die Erziehungswissenschaften stützen, um uns bei der Durchführung und Begleitung von Veränderungen durch Akteure aus der Praxis zu helfen. Wir werden mehrere Autoren heranziehen, um einen epistemologischen Blick zu erhalten, den wir mit der doppelten Haltung des Praktikers und Forschers in Beziehung setzen werden, um eine Interventionsforschung durchzuführen. Die Ergebnisse der monozentrischen explorativen Umfrage zeigen, dass die Unruhe intubierter, beatmeter Patienten bei den Pflegenden Angst auslöst. Das Sevrabilitätsprotokoll und die Praktiken sind zwischen den beiden Wiederbelebungsstationen nicht harmonisiert. Einer der wichtigsten Handlungsansätze, den die Gruppe der referierenden Akteure in Betracht zieht, wäre die Einführung eines protokollierten Entwöhnungsinstruments, um die Pflegepraktiken in den beiden Stationen zum Wohle der Patienten und des Pflegepersonals zu harmonisieren.

A Structured Elicitation Approach to Identify Technology-Based Challenges

Les résultats de l'enquête exploratoire monocentrique révèle que l'agitation des patients intubés ventilés induit de l'anxiété chez les soignants. Le protocole de sevrabilité et les pratiques ne sont pas harmonisés entre les deux unités de réaniamtion. Une des principales pistes pour l'action que le groupe d'acteurs référents envisage serait la mise en place d'un outil protocolé de sevrabilité pour harmoniser les pratiques de soins dans les deux unités pour l'intérêt des patients et des soignants.
